Everything we do in life is a risk we take, lets minimize those risks by being clear about all medical conditions & medications, previous treatments and recent procedures. We want the best for you and will guide you where we can if we cannot assist you with your skin condition or concern. I, the undersigned, hereby acknowledge that I have chosen to undergo treatment/s at Living Skin (Pty) Ltd. The treatment/s will be administered by a qualified practitioner. I acknowledge that I have been informed of the nature of the treatment and the treatment protocols and need to strictly adhere to the instructions. If I do not adhere to the treatment protocols my results may be jeopardized. I have informed Living Skin of any medical history as well as medication including Roaccutane etc. and I have advised my practitioner if I’m prone to cold sores. I acknowledge that there are no guarantees in terms of results and that there is a possibility that the final result might not meet my expectations. Furthermore, as is the case with any depigmentation treatment or acne treatments, there exists a possibility that pigmentation and acne can return. I hereby absolve Living Skin Clinic, its agents, the suppliers and distributors of the treatment/s and products of any adverse effects that might result from the treatment and the use of the accompanying products. I understand that results vary from patient to patient and that if I have any concerns I should contact Living Skin Clinic. I have read the above statement and understand it. I have time in my consultation to ask questions and have all my questions answered satisfactorily. I accept the risks and possible complications of the procedure/s.
